ADMISSION REQUIREMENTS

Children Should Be Between 9 months To 5 Years Old
Must Have an Up-To-Date Health Examination
Must Have an Up-To-Date Record Of Immunizations On File
Must Provide a Copy Of The Child’s Original Birth Certificate
Parents Must Have a Signed Acknowledgment Of Receipt Of The Summary Of Licensing Standards For Day Care Centers
Contact Information For Parents/Guardians and the Child’s Doctor
Record Of Receipt Of a Parent Handbook
Must Have a Pick-Up List Of All People Authorized To Pick Up Your Child From Day Care

TUITION PAYMENTS

Tuition payment is due on the first day the child starts. Due to school budgeting we can not allow tuition payments to be late. Under normal circumstances, if payment becomes two weeks overdue your child may not be able to return to the center until all financial obligations are met. The regular tuition payment will be charged if your child comes to the center any portion of the week. If your child is absent the entire week, 1/2 the regular tuition must be paid. If the center is given a written notification two weeks prior to a family vacation no tuition will be due that week. One week vacation per year. Please contact the center for more information on tuition policies.

MEDICAL RECORDS
All current Medical records & immunizations must be submitted to center upon enrollment.
